Holzer Clinic now offers you the ability to make more informed decisions about your health with the FollowMyHealth@Holzer patient portal.

The patient portal is a secure website where you can schedule, change, or cancel an appointment, or view your health history, lab results, medication lists and drug allergies. It provides prescription renewal and pre-registration services and allows you to communication securely with your doctor. But more than that, it’s your own personal record of your health, making it easier for you to be more actively involved in managing your own care.

FollowMyHealth@Holzer is free, secure, and convenient. This self-service health management tool can improve your interactions with your physician and ultimately give you the ability to make more informed decisions about your health! FollowMyHealth@Holzer is built on the idea that managing your healthcare information can be more intuitive, more efficient and maybe even fun.

How do I sign up for FollowMyHealth@Holzer?

The most secure means of registering for a FollowMyHealth account is to ask any receptionist about this service during your next visit to Holzer Clinic. The receptionist will collect the necessary information and assist you with completing a registration form. Then within two days you will receive an ‘invitation’ by email that will automatically connect your FollowMyHealth account to your medical information.

Frequently Asked Questions

When I receive an email invitation from FollowMyHealth@Holzer, how long will the link in the email be valid?

Your invitation will be valid for 30 days.

Can a parent/guardian be set up to manage an account for a child?

Parents can be granted proxy access to their children’s chart but you may be asked to provide supporting documentation of guardianship in some cases. Due to state laws only limited access can be provided for children between the ages of 15 and 18.

Can I be granted access to manage an account for a non-minor family member, such as elderly parent ?

We can grant what’s called “proxy” access, which allows a user to manage another individual’s medical information. Proxy access requires supporting documentation, such as a power of attorney. Requests for proxy access may require additional time to complete. This is to provide us with the time to perform the necessary review of documentation. A user can grant proxy access on their own to another registered user through the “Settings” menu inside FollowMyHealth@Holzer.

Is there a timeout setting?

Yes, users will be automatically logged out of FollowMyHealth@Holzer after 10 minutes of inactivity.

Why do I need a FaceBook, Google, Yahoo or Windows Live-ID account?

FollowMyHealth@Holzer uses these accounts to provide the necessary authentication technology to log in to the application.

Can I remove a connection in FollowMyHealth@Holzer?

Yes, you can remove a connection with a medical institution which will disabled further communication of medical data but the historical data will remain. This can be done within the application using the connections tab by removing the organization from which you no longer want information.
